Paradigm Shift to Promote a Revolution of Treatment of Schizophrenia to Achieve Recovery

Abstract

Active schizophrenia is characterized by psychotic symptoms that can include delusions, hallucinations, disorganized speech, trouble with thinking, and a lack of motivation. The introduction of antipsychotics in the 50ties was called a revolution for the treatment of schizophrenia and psychosis and is now a cornerstone of treatment. The effects are considered well documented. Guidelines suggest that all patients be offered antipsychotics. Nearly all patients are medicated. Concerns have been raised about non-responders, and there is no evidence for long-term effects. The validity of the diagnosis of schizophrenia according to the Diagnostic and Statistical Manual Mental Disorders (DSM) is controversial and discussed. Patients experience side effects and low benefits and try to stop. This is considered non-conformance, which can lead to relapse. Therefore, caregivers consider medication necessary and use forced drugging. Antipsychotics reduce psychotic symptoms. New patient laws in many countries aim to promote recovery.

The United Nations Special Rapporteur on the Right to Health has called 2019 for «World needs a "revolution" in mental health care». "There is now unequivocal evidence of the failures of a system that relies too heavily on the biomedical model of mental health services, including the front-line and excessive use of psychotropic medicines, and yet these models persist". However, this proposal is highly controversial for clinicians.

World Health Organization (WHO) followed up 2021: “New WHO guidance seeks to put an end to human rights violations in mental health care”. "This comprehensive new guidance provides a strong argument for a much faster transition from mental health services that use coercion and focus almost exclusively on the use of medication to manage symptoms of mental health conditions to a more holistic approach that takes into account the specific circumstances and wishes of the individual and offers a variety of approaches for treatment and support".

This is highly controversial; however, the United Nations High Commissioner for Human Rights (OHCHR) supported the WHO proposal with a guideline to reform legislation in order to end human rights abuses and increase access to quality mental health care. A realistic path to implement WHO treatment improvements seems to need the courage of legislators to follow OHCHR suggestions and a shift of paradigm.

Case Against Germany: No Right to Information Law in Bavaria Breaches Right to Freedom of Expression

The Centre for Law and Democracy (CLD) has filed a human rights complaint (communication) against Germany with the United Nations Human Rights Committee. The communication alleges that Germany has failed to meet its obligations to respect freedom of expression under Article 19 of the International Covenant on Civil and Political Rights because the German state of Bavaria does not have a right to information (RTI) law. CLD’s client, Mr. Walter Keim, was refused access to public interest documents related to his human rights activism. The communication has now been registered and sent to Germany, which has six months to respond.

Falsifisering av psykiatriens myter tvinger frem et paradigmeskifte og en revolusjon i behandling

Psykiatrisk praksis bygger på at neste alle med diagnose psykose medisineres basert på myten "hos det store flertallet bidrar medisiner til symptomlette, funksjonsbedring og høyere selvrapportert livskvalitet." «Antipsykotiske midler ... førte til en revolusjon i behandlingen» (Legetidsskrift, 12.05.2017) med feiloppfatninger om langtidsmedisinering, akutt- og vedlikeholdsmedisinering, at medisinfri behandling er uetisk og ignorering av menneskerettigheter.

Innspill til neste oppdatering av samvalg psykose som er 1.7.24 med råd «Det er vanlig å bruke antipsykotiske legemidler i 2 år ved førstegangs psykose» og «Ved nye psykoseepisoder er det vanlig å bruke medisiner i 5 år».

Sammenfatning:

86% av behandlere/psykiatere tror på at langtidsmedisinering er nyttig (1), selv om det ikke finnes evidens for det (Sohler et al. 2015, Leucht et al. 2012)

Akutt symptomreduksjons effekt er liten (Leucht et al. 2017), usikker (FHI 2019) og ikke godt dokumentert (2)

Vedlikeholdsmedisinering er usikker, ikke godt dokumentert pga. mulig abstinens, tidsavhengig (Leucht et al. 2012, Fig 5), bare korttidsvirkende (Schlier et al. 2023) og fraværende etter 2 år, (2)

Tilfriskning (recovery) som loven krever gikk ned: «17,7 % i studier mellom 1941 og 1955, 16,9 % i 1956–1975, 9,9 % i 1976–1995 og 6,0 % i studier etter 1996» ifølge (Jaaskelainen et al. 2013)

For 9% av pasientene usikker nytteeffekt med god akutt symptomdemping og at 94% (kilder) opplever bivirkninger fører til at «opp til 93%» slutter med nevroleptika (FHI rapport 8-2009). Bare tvang kan oppnå at nesten alle med diagnose psykose medisinernes

Psykiaterne har en kunnskapsresistent vrangforestilling at antipsykotika fører med «stor sannsynlighet (...) til helbredelse eller vesentlig bedring» da

Medisinfri behandling ønsket av pasientene har etterhvert mange studier, effekten er stor og stadig bedre dokumentert (3)

Menneskerettigheter setter krav til høy faglig standard, reduksjon av tvang og avskaffelse av tvangsmedisinering.

Ifølge FN rapporteur for rett til helse trenger psykiatrien en revolusjon for å unngå eksessiv overmedisinering (11). WHO (12) og FNs høykommissær for menneskerettigheter foreslår lovendringer (13) som betyr et paradigmeskifte og revolusjon (14)
